The Compass You Need for the Path Ahead
When a life-limiting diagnosis enters a family, the world can feel like it’s spinning. You have doctors for the body, but you need a guide for the journey. My name is Rachael Paje, and I provide the steady presence, clinical wisdom, and emotional navigation families need to find peace in life’s final chapters.

20 Years of Clinical Expertise:
Two decades as a Palliative RN means I bring "professional eyes" into the home to ensure comfort and safety.
Lived Experience:
Having nursed my own husband through his end-of-life journey, I don’t just understand the medical side—I understand the heart of a caregiver.
A Holistic Approach:
I believe that "living your best life until the end" happens best in the fresh air. I bridge the gap between clinical care and the healing power of nature.
-
Clinical Navigator:
Acting as a "Medical Translator" to help you understand care plans and diagnoses.
Advocacy during doctor appointments and hospital transitions.
Home safety audits and symptom management education.
Legacy Coaching:
Facilitating "Unfinished Business" conversations and legacy projects.
Creating "Vigil Plans" to ensure the final days are peaceful and personal.
Emotional support for spouses, children, and grandchildren.
Nature & Movement:
Leading "Grief in Motion" walks on local trails to process emotions.
Tailored outdoor sessions to reduce caregiver burnout and isolation.
Bringing the "outdoors in" for those with limited mobility.

Working Alongside Your Medical Team
At Changing Tides, I believe the best care happens as a team, not a silo. My role is to act as a clinical bridge, working alongside your Family Physician, Palliative Specialists, and Health Authority Nurses—not instead of them. While I bring 20 years of RN expertise to our sessions, I do not prescribe medication or alter medical orders. Instead, I provide the dedicated time, advocacy, and coaching that allows your existing care plan to be navigated with peace and clarity.
Navigation for All End-of-Life Options
Part of my commitment to your family is ensuring you have clear, non-judgmental information about all legal pathways in British Columbia. This includes providing education and emotional coaching regarding Medical Assistance in Dying (MAiD).
Please note that as a Consultant and Doula, I do not perform formal MAiD assessments or administrations; however, I am here to help you understand the provincial process, coordinate with your medical providers, and provide a steady, supportive presence for your family throughout the entire journey.
