The Anchor Session (In-Home Discovery)
A comprehensive in-home consultation including:
Clinical Environment Audit: Assessment of home environment to provide recommendations for improving patient comfort and safety .
Medical Navigation: A walk through of the doctor’s care plans including the diagnosis and prognosis, and the purpose of individual medications.
The Nature Scan: Studies show time spent in nature improves life satisfaction and may reduce some symptoms. We’ll identify ways to bring nature into the patient’s daily life.
The Path Forward: Your written plan and resource guide summarizing my review.
Investment: $400
The Final Vigil (End-of-Life Support)
Intensive, high-touch presence during the active dying process to ensure a peaceful "full circle" experience.
Vigil Planning: Creating a sacred space (music, lighting, readings).
Respite for Caregivers: Professional RN-level presence so family can rest.
Immediate Guidance: Support with post-death logistics and early bereavement.
Customized Packages Starting at: $1,500
Navigational Coaching (Monthly Support)
Consistent, RN-led support to ensure no one in the family walks this path alone. Up to four, 60 minute sessions per month to address your priorities in person as well as support by text or email. Patients may wish to discuss:
Clinical update: check-ins to review or update your plan and discuss developments.
Time in nature: "Grief in Motion" nature walks.
Legacy Projects: Dedicated time for recording stories or organizing "unfinished business."
Physician appointments: I can join you at key appointments to take notes, debrief discussions or help ensure your voice is heard.
Patient advocacy: If you are not receiving the care you need, I can strategize solutions with you and help you get the best outcome our medical system can offer.
